Ashwagandha for Stress and Skin: What the Studies Actually Show

Ashwagandha has become one of the most talked-about "adaptogen" supplements in stress and skincare marketing, credited with everything from lower cortisol to clearer skin to better sleep. Some of that reputation has real research behind it. Ashwagandha, an herb used for centuries in Ayurvedic medicine and derived from the Withania somnifera plant, has been studied in a growing number of randomized, placebo-controlled trials that specifically measure its effect on cortisol and perceived stress. The skin-specific claims layered on top of that research, however, are a separate and much thinner body of evidence, and it is worth pulling the two apart.

The cortisol evidence is the strongest part of the story. A 2024 systematic review and meta-analysis published in BJPsych Open pooled seven trials on cortisol and six on perceived stress across 488 adults and found that ashwagandha supplementation was associated with a significant reduction in cortisol compared with placebo. Individual trials support this: a 2019 randomized, double-blind, placebo-controlled study in Cureus gave healthy adults either 250 mg or 600 mg of ashwagandha root extract daily and found significant drops in serum cortisol at both doses compared with placebo, alongside improved self-reported sleep quality. That same meta-analysis, though, found no significant effect on perceived stress ratings, meaning people's measured cortisol moved even when they did not necessarily feel less stressed, an easy-to-miss distinction.

The skin side of the pitch is where marketing runs further ahead of the evidence. There is essentially no published research directly testing whether oral ashwagandha supplementation improves stress-related skin conditions like acne or flares in a controlled trial. What does exist is a separate line of research entirely: a placebo-controlled study of a topical, 8 percent ashwagandha root extract lotion, applied directly to facial skin in adults with visible photoaging, reported improvements after 60 days in measures including wrinkles, pore appearance, pigmentation, and skin hydration. That is real data, but it describes a topical, antioxidant-type cosmetic effect, not a cortisol-mediated one, and it says nothing about whether swallowing a capsule changes anyone's skin.

This is the key place to separate what is proven from what is sold. Proven, with reasonable trial support: oral ashwagandha measurably lowers cortisol across several controlled studies. Marketed, without direct trial support: that this cortisol reduction translates into clearer skin, calmer rosacea, or fewer breakouts for a given person. The theoretical mechanism, lower cortisol reducing androgen-driven oil production and inflammation, is biologically plausible given what is known about cortisol and skin more broadly, but plausible is not the same as demonstrated, and no trial has connected the dots from ashwagandha capsule to a measured skin outcome the way the topical lotion study did for its own separate application.

Trial duration is also a real limitation worth naming: most ashwagandha and cortisol studies run six to eight weeks, and none of the trials feeding the major meta-analyses have run meaningfully longer, so the long-term effect and safety picture is still incomplete. Ashwagandha can also interact with thyroid medication, blood pressure medication, and immunosuppressants, and is generally not recommended during pregnancy without medical supervision, which is a reason to treat it as a supplement worth discussing with a doctor rather than an automatically safe daily add-on.

Taken together, ashwagandha is one of the more evidence-backed adaptogens for cortisol specifically, which is not nothing. But the leap from "lowers cortisol in an eight-week trial" to "clears your stress skin" is currently more marketing than research, and closing that gap would need trials that actually measure skin outcomes in people taking the supplement, not just their hormone levels.
