How Doctors Actually Test Cortisol (and Why At-Home Kits Are Different)

Direct-to-consumer "cortisol test" kits, saliva swabs, finger-prick cards, subscription hormone panels, have turned a specialized piece of endocrinology into something that looks like a quick wellness check. It is worth being clear about what actually happens when a doctor orders cortisol testing for a real suspected medical issue, because the clinical process is considerably more specific than most at-home products let on.
Cortisol can be measured in blood, saliva, or urine, and the Endocrine Society's clinical practice guideline on diagnosing Cushing's syndrome lays out which of these are considered reliable starting points: a 24-hour urine free cortisol collection, a late-night salivary cortisol sample, or a low-dose dexamethasone suppression test, in which a patient takes a small dose of the steroid dexamethasone and a doctor checks whether it appropriately suppresses the body's own cortisol production the next morning. Each method captures something different: urine collection reflects total cortisol output over a full day, a single blood draw reflects one moment, and saliva reflects the unbound, biologically active cortisol circulating at a specific time.
Timing is not a minor detail in any of this, it is often the entire point of the test. Cortisol follows a predictable daily rhythm, peaking shortly after waking and dropping to its lowest point around midnight, so a late-night saliva sample is specifically useful because cortisol should be low then. If it is not, that mistimed elevation is itself diagnostically meaningful. A random daytime blood draw without that context tells a doctor much less, which is one reason clinicians are careful about exactly when a sample is collected relative to waking time, meals, and stress.
This is where most consumer at-home kits diverge from clinical practice. A 2023 study comparing salivary cortisol to serum cortisol against the gold-standard ACTH stimulation test in children found that while salivary cortisol can correlate reasonably well with blood levels when measured with validated laboratory methods, the immunoassays used in many commercial kits have known issues with specificity and can show meaningful differences between assay brands. Consumer kits also typically report a single result against a generic reference range rather than the age-specific, method-specific, precisely-timed protocol that clinical guidelines are built around. No major medical body currently endorses at-home cortisol kits as a substitute for laboratory-based diagnostic testing.
None of this means at-home testing is worthless as a general curiosity, or that saliva as a sample type is inherently unreliable, laboratory-grade salivary cortisol testing is a legitimate, guideline-recognized clinical tool. The gap is specifically between a validated lab process interpreted by an endocrinologist against a person's symptoms, and a consumer kit that returns one number with a generic "normal" range and no clinical context attached to it.
If there is a real concern about a cortisol-related condition, persistent unexplained fatigue, dramatic weight changes, unusual bruising, or other symptoms suggestive of Cushing's syndrome or adrenal insufficiency, the appropriate next step is a conversation with a doctor about which specific, correctly-timed test fits the situation, not a mail-in kit ordered on a whim. The at-home versions can be an interesting personal data point, but they are not built or validated to answer the medical question they are often marketed as answering.
