Why Stress Triggers Eczema and Rosacea Flare-Ups

People with eczema have said for years that stress makes their skin worse. A survey by the National Eczema Society found that many patients rated stress as the single biggest trigger of their flare-ups, ahead of foods, weather, or fabrics. Until recently, though, the biological explanation for that connection was mostly a gap: researchers could observe the correlation without a clear mechanism for how a psychological state translates into inflamed skin.
That gap narrowed considerably with a 2026 study, published in the journal Science, from researchers at Fudan University. Working with mouse models of atopic dermatitis, the more clinical name for eczema, the team traced a specific neural pathway that responds directly to stress. A group of nerve cells, identified by a marker called Pdyn, activate under stress and release a signaling protein called CCL11. That protein recruits eosinophils, a type of immune cell, directly to the skin. Once there, those cells trigger the redness, swelling, and itch of a flare-up. Skin biopsies from human patients with higher self-reported stress showed correspondingly higher eosinophil levels, connecting the mouse mechanism back to real patient data.
This matters because it reframes stress-related flare-ups as a direct neuroimmune signal, not just a vague, secondary side effect of feeling bad. The nervous system is not passively responding to inflammation that started elsewhere; in this pathway, stress signaling is an active trigger of the immune response itself. The researchers who led the study have said they are now investigating whether a similar nerve-to-immune-cell circuit might explain stress-driven flares in other conditions, including rosacea, chronic hives, and psoriasis, though that work is still ongoing.
Rosacea already has substantial, separate evidence for a stress connection, gathered through patient surveys rather than this specific neural pathway. A National Rosacea Society survey found that a majority of rosacea patients reported experiencing a stress-related flare at least monthly. Research into the condition has also found that people with rosacea show a measurably stronger nerve, blood flow, and sweating response to heat and stress than people without it, pointing to an oversensitive sympathetic nervous system response as part of what makes rosacea-prone skin more reactive in general.
For someone managing either condition, the practical value of this research is less about a new treatment, none exists yet based specifically on blocking this pathway, and more about validation and where to focus effort. Stress management is not a vague wellness add-on for eczema or rosacea; it is targeting a documented, specific contributor to flare frequency. Identifying personal stress patterns, working with a dermatologist on both the physical and stress-related sides of the condition, and treating stress reduction as part of the actual treatment plan, not just a nice-to-have, is a reasonable response to this evidence.
